Quick answer
Waiver forms are documents customers sign to accept the risks of an activity and release a business from certain claims. This library has 32 free waiver templates across health and beauty, sports and fitness, adventure parks, rentals and kids activities. Each template includes the waiver text plus the fields and health questions that type of business needs, and every one can be published as an online waiver in one click.
What a waiver form is, and why templates help
A waiver form, also called a liability waiver, release form or consent form depending on the setting, is a short agreement in which a customer confirms they understand the risks of an activity and give up the right to make certain claims against the business. In regulated services like tattooing, aesthetics or dental care, the same document often doubles as informed consent.
Most waiver forms share a skeleton: what the activity is, what can go wrong, what the customer releases, what health information matters, and a signature. What changes between industries is the detail. A climbing gym needs belay rules. A tattoo studio needs blood thinner and allergy questions. A trampoline park needs a fast way for one parent to sign for four children.
That is the job of a waiver template. It gives you the skeleton plus the industry detail, so you edit a strong draft instead of guessing what to include. You still make it your own, and you should still have a local attorney review the final wording, but you start much closer to the finish line.
Types of waiver forms, and which one you need
| Waiver form type | What it does | Typical users |
|---|---|---|
| Liability waiver | Customer accepts activity risks and releases ordinary negligence claims | Gyms, parks, tours, rentals |
| Release of liability | Settles or releases claims, sometimes after an incident | Insurers, event organizers |
| Informed consent form | Patient or client agrees to a procedure after risks are explained | Tattoo, aesthetics, dental, physio |
| Guardian consent waiver | A parent signs for a minor and authorizes care | Camps, kids parties, schools |
| Rental agreement with waiver | Combines hire terms, damage responsibility and risk release | Bikes, boats, tools, equipment |
| Media release | Permission to use photos or video of a person | Events, studios, schools |
Many businesses need two: a main waiver for the activity and a separate, optional media release. Keeping photo consent separate is fairer to customers and avoids privacy problems.

Best practices when customizing waiver forms
Replace every generic phrase with your real activity. "Activities" means nothing to a judge. "Zipline rides up to 300 meters" means a lot.
Keep one waiver per distinct activity. A gym with a kids club needs two waivers, not one stretched to fit both.
Ask only health questions you will act on, and flag the answers staff must see.
Leave the exclusion for gross negligence in place. Overreaching clauses can make a whole waiver look unfair.
Turn on guardian signing for any activity minors might join.
Set an expiry, usually 12 months, so long-term customers sign your current version.
Add translated text if many of your customers speak another language. A waiver nobody understood is weak.
Have your final customized version reviewed by a local attorney once, then reuse it confidently.
An example scenario: one business, two templates
A family entertainment center runs a trampoline arena on weekdays and hosts birthday parties on weekends. The owner first tried to use one waiver for both and quickly hit problems. Party guests were children whose parents were not always present, and the trampoline wording said nothing about food, allergies or who could collect each child.
She started two waivers from the library: the trampoline park template for jump sessions and the kids play center and party template for events. The trampoline version kept its jump rules and added a sock and age-group rule. The party version gained an allergy question, flagged, and an authorized pickup field.
Party hosts now send one link to invited parents a week before. Each parent signs for their own child from home, and the party staff see allergy flags before the cake comes out. Jump session customers use the other waiver at the kiosk. Two short documents replaced one confusing one.

What each category of waiver template focuses on
Health and beauty waiver forms
These work more like informed consent than a pure liability release. The focus is on disclosing procedure risks, capturing allergies, medications and pregnancy, and recording that aftercare instructions were given. Several, like tattoo and aesthetics, also require age verification.
Sports and fitness waiver forms
Built around assumption of risk and a health screen. Gym templates include PAR-Q style questions about heart conditions and chest pain. League and team templates add season-long validity and guardian signing for junior players.
Adventure and park waiver forms
These carry the most detailed risk descriptions, because the risks are physical and obvious: falls, impacts, heights and speed. Weight, height and age limits appear as fields, and fast family signing matters because most visitors arrive in groups.
Rental and service waiver forms
Rental templates combine a waiver with contract terms: return dates, damage and theft responsibility, and safe use. Service templates like venue hire and pet grooming focus on conduct rules, deposits and specific authorizations such as calling a vet.
Kids and education waiver forms
Always signed by a parent or guardian. They add authorized pickup lists, allergy and medication details, emergency treatment authorization and, for camps and trips, overnight or travel consent. Media consent is usually a separate, optional tick box.
Not sure where you fit?
Pick by your highest-risk activity, not your business label. A cafe with a climbing wall needs the climbing template. A salon offering microblading needs the aesthetic consent, not the hair salon one.
How often to update your waiver forms
A waiver form is not a one-time task. Review it at least once a year, and immediately whenever something in your business changes: a new activity, a new location, a new age group, new equipment, or a change in local law your insurer or attorney flags.
When you update an online waiver, past records keep the exact text each customer signed, so nothing is lost. New signers see the new version. If the change is significant, for example adding a new activity to the release, set existing waivers to expire so regular customers sign the updated version on their next visit. With paper, that would mean reprinting everything and hoping old stacks get thrown away. Online, it is one edit.
Keep a short change log of what you changed and when. If a claim ever turns on which wording a customer agreed to, a clear version history, plus the stored text on each signed record, answers the question in minutes.
Common waiver form terms, explained in plain words
| Term | What it means |
|---|---|
| Assumption of risk | The signer confirms they know about specific dangers and choose to take part anyway. |
| Release | The signer gives up the right to sue for certain claims, usually ordinary negligence. |
| Indemnity or hold harmless | The signer agrees to cover costs if their own actions cause a claim against the business. |
| Ordinary negligence | A normal mistake or oversight. Most waivers can release this. |
| Gross negligence | Reckless disregard for safety. Most courts will not let a waiver release this. |
| Severability | If one clause is struck down, the rest of the waiver still stands. |
| Guardian consent | A parent or legal guardian agreeing on behalf of a minor. |
Every template uses these terms the same way, so once you understand them you can read and edit any waiver form in the library with confidence.
